Billing & Subscription Authorization

1. Authorization for Recurring Charges
By enrolling in a membership or subscription plan offered by Daily Slim Down LLC ("Daily Slim Down", "we", "us", or "our"), you authorize Daily Slim Down (and its payment processor) to charge your designated payment method on a recurring basis for the plan you selected, plus any applicable taxes and any clinical, medication, lab, or shipping charges that are billed separately.
2. Plans & Pricing
The current list of plans, the services included in each plan, the recurring price, the billing cadence (typically monthly), and the items that are billed separately is published on our public pricing page and presented to you at checkout. The price and plan you see at checkout is the price you authorize.
Some plans include only platform access and provider services and bill medication separately through the dispensing pharmacy or your insurance. Other "all-inclusive" plans bundle the cost of certain medications (which may include compounded medications prepared by licensed 503A or 503B compounding pharmacies). Compounded medications are not FDA-approved; see our Telehealth Consent for additional disclosures.

3. Auto-Renewal & Cancellation Notice
Your subscription will automatically renew at the end of each billing cycle at the then-current price for your plan, until you cancel. We will charge your payment method on the same calendar day each month (or the closest available date in shorter months) until you cancel.
You may cancel at any time, effective at the end of the current billing cycle, by:
- Selecting Cancel Membership in your account settings;
- Emailing [email protected]; or
- Sending a secure message to your provider or care team.
Cancellation stops future charges; you retain access through the end of the paid period. Cancellation does not eliminate any outstanding balance, including charges for medication already shipped or services already rendered. California residents may cancel by the same online means in which you signed up; we will provide a confirmation of cancellation by email.
4. Free Trials, Promotions & Discount Codes
Promotional pricing, free-trial offers, and discount codes are subject to the terms presented at the time of the offer. Unless stated otherwise, free trials automatically convert to a paid subscription at the end of the trial period at the then-current price unless you cancel before the trial ends. Promotions cannot be combined and have no cash value.
5. What's Included vs. Separately Billed
Typically Included in an Essentials Plan
- Platform access (web and mobile);
- Provider consultations and secure messaging within the plan's scope;
- Care coordination and AI-assisted features;
- Prescription writing within the scope of your visit; and
- Bundled medication and shipping for "all-inclusive" plans, as described at checkout.
May Be Billed Separately
- Brand-name medications dispensed by retail or mail-order pharmacies;
- Laboratory tests and diagnostic services;
- In-person visits or referrals;
- Specialty consultations beyond the scope of your plan;
- Expedited or special shipping; and
- Services not included in your selected plan.
6. No Guarantee of Prescription
Membership and subscription fees pay for platform access and clinical evaluation — not for any specific medication.A prescription will be issued only when, in the provider's professional judgment, treatment is medically appropriate, safe, and legal. If your provider determines a prescribed therapy is not appropriate for you, you may request a prorated or full refund of bundled medication fees within 30 days as described in Section 9.
7. Payment Methods & Failed Payments
We accept major credit and debit cards, and (where applicable) HSA/FSA cards. Card data is tokenized by our PCI-DSS-compliant payment processor and is not stored on our servers. You are responsible for keeping your payment information current.
If a payment fails we will attempt to retry the charge and notify you by email or in-portal message. If payment is not received within a reasonable cure period, we may suspend or terminate your access, withhold prescription refills, and pursue collection. Delinquent accounts may be referred to a third-party collection agency or reported to credit bureaus to the extent permitted by law.
8. Price Changes
We may change pricing from time to time. If we increase the recurring price for your plan, we will give you at least 30 days' advance notice by email or in-portal message. The new price will take effect on your next billing cycle. If you do not agree with the price change, you may cancel before it takes effect; continued use after the effective date constitutes acceptance.
9. Refund Policy
Except as required by law, fees are generally non-refundable once charged. We will issue refunds for:
- Verified billing errors or duplicate charges;
- Services not rendered due to our error;
- Medication that was paid for but not shipped;
- Within 30 days of enrollment, if your provider determines that prescribed therapy is not medically appropriate; and
- Where required by law, including any applicable cooling-off rights.
Refund requests must be submitted to [email protected] within 30 days of the charge in question. Refunds are typically issued to the original payment method within 7–14 business days.
10. Insurance & HSA / FSA
Memberships are typically not covered by health insurance and we do not bill insurance directly. Many of our services may be eligible for HSA/FSA reimbursement and we can provide itemized receipts on request. You are responsible for all charges regardless of whether you obtain reimbursement.
11. Chargebacks & Disputes
If you believe a charge is incorrect, please contact us before initiating a chargeback so we can resolve the issue directly. Initiating a chargeback for a valid charge may result in suspension of your account. We reserve the right to challenge chargebacks we believe to be invalid using transaction records, audit logs, and shipment confirmations.
12. Taxes
Posted prices do not include applicable sales, use, or similar taxes, which will be calculated and added at checkout based on your billing or shipping address.
13. Plan Changes
You may request to upgrade, downgrade, or change plans at any time. Changes typically take effect at the start of the next billing cycle. Switching plans may be subject to provider approval based on medical appropriateness and may affect bundled medication eligibility.
